Our Primary to Secondary Transition Portal is a secure, online, platform used to transfer data for year 6 students, from their primary school to their secondary school. Using the portal gives schools a standardised means to share holistic, detailed student information, identifying vulnerable pupils as early as possible in the transition process. The Transition Portal allows primary schools to operate one, consistent and common approach, this has shown to improve the dialogue between primary and secondary schools at scale.
